The HMNZS Manawanui, a Royal New Zealand Navy vessel, grounded and sank off the coast of Samoa due to a series of human errors, according to an interim report released by Chief of Navy Rear Admiral Garin Golding. The incident occurred on October 5 while the ship was conducting survey operations.
The report states that the ship’s autopilot was not disengaged as it should have been, leading the crew to believe the vessel’s failure to respond was due to issues with the thruster control. Golding noted that standard procedures were not followed, which should have prompted the crew to confirm the ship was operating manually. The failure to perform this check resulted in the ship maintaining its course towards land until it hit the reef.
Golding emphasized that a deeper investigation is ongoing as part of a Court of Inquiry, expected to conclude in the first quarter of next year. This inquiry will examine the exact circumstances that led to the sinking and determine accountability. A disciplinary process for three key personnel involved in the accident will follow the inquiry’s completion.
In response to the incident, Golding mentioned that audits have been initiated across the Fleet to enhance training and risk management, and improvements to relevant procedures are underway. The Navy has expressed gratitude for the support received from various organizations, including the Transport Accident Investigation Committee and the Royal Australian Navy.
Defence Minister Judith Collins described the findings as “extremely disappointing,” but acknowledged the Navy’s transparency in taking responsibility. She reminded the public that no lives were lost in the grounding, which is a positive aspect of an otherwise unfortunate event.
Golding confirmed that salvage operations are planned to begin around December 16, depending on weather conditions, with the objective of completing the operation before the cyclone season. While the $100 million ship was insured for salvage, it is not covered for replacement.
