Investigation Reveals Navy Ship Grounding Breaches

The Republic of Fiji Military Forces (RFMF) has concluded the investigation into the grounding of the Republic of Fiji Navy Ship (RFNS) PUAMAU at the Fulaga Reef in the Lau group. According to Major-General Ro Jone Kalouniwai, the Commander of the RFMF, the investigation conducted by the ‘Board of Inquiry’ (BOI) revealed significant deficiencies in procedures and standards within Fiji Navy’s maritime operations, especially in navigation practices aboard the ship.

The findings of the BOI indicate substantial breaches, negligence, and non-adherence to professional and accepted navigation practices and mariner skills. The report further highlights that the Commanding Officer and his command team did not comply with best bridge management processes, particularly in applying safety measures under confined and hazardous environmental conditions near Fulaga Island.

Kalouniwai noted that the Commanding Officer and his bridge team neglected best mariner practices associated with using navigation equipment for situational awareness and decision-making.

In response to these findings, the BOI has recommended several measures to address the identified deficiencies and enhance future maritime operations’ safety. These measures include holding the Commanding Officer accountable for the grounding, ensuring comprehensive patrol briefings and navigational planning, implementing clear Standard Operating Procedures (SOPs), providing all seaman officers with navigation books, improving situational awareness for commanding officers, and reviewing bridge procedures during entry into confined waters.

Kalouniwai emphasized the RFMF’s commitment to upholding the highest standards of professionalism and safety within the Fiji Navy. By implementing the BOI’s recommendations, they aim to prevent similar incidents and ensure the continued efficiency and effectiveness of Fiji’s maritime operations.
