Fiji Military Commanding Officer Held Accountable: Investigation Findings Unveiled

The Republic of Fiji Military Forces Board of Inquiry has held the Commanding Officer and his command team accountable for the grounding of Fiji’s patrol boat RFNS Puamau. Major General Ro Jone Kalouniwai outlined several recommendations in a statement, including holding the Commanding Officer responsible for the incident, ensuring comprehensive patrol briefings and navigational planning, implementing clear Safe Seaward Departure (SSD) procedures, providing seaman officers with Navigation Books, enhancing situational awareness for commanding officers, and reviewing bridge procedures during entry into confined waters.

Maj-Gen Kalouniwai noted that the investigation revealed critical deficiencies in procedures and standards within the Fiji Navy’s maritime operations, particularly concerning navigation practices on the ship. The findings of the Board of Inquiry highlighted significant breaches, negligence, and lapses in the application of professional bridge management processes by the Commanding Officer and his team, especially under the challenging environmental conditions near Fulaga Island.

The inquiry further noted that the Commanding Officer and his bridge team neglected certain best mariner practices and processes related to the use of navigation equipment for situational awareness and decision-making.

Maj-Gen Kalouniwai emphasized the Republic of Fiji Military Forces’ commitment to maintaining the highest standards of professionalism and safety within the Fiji Navy. By implementing the Board of Inquiry’s recommendations, the RFMF aims to prevent similar incidents in the future and ensure the continued efficiency and effectiveness of maritime operations.
