Fiji Considers Needle Exchange Program: A Step Towards Harm Reduction?

The Ministry of Health is exploring the possibility of introducing a needle exchange program as part of its Counter Narcotic Strategic Plan aimed at harm reduction. This initiative, as explained by Acting Chief Medical Advisor Dr. Luisa Cikamatana, seeks to tackle the issues faced by intravenous drug users in Fiji while also drawing from successful international practices.

In a recent interview, Dr. Cikamatana emphasized the potential advantages of creating designated areas where individuals can safely obtain syringes and needles for drug use. She pointed out that in other countries, such initiatives offer a controlled and secure environment for users.

“Abroad, there are specialized locations where individuals can come in to receive syringes and needles,” Dr. Cikamatana noted. “It’s a safe space for them to use before they leave.”

She also highlighted the necessity for further evaluation within the local context, stressing that while similar programs have thrived elsewhere, their acceptance in Fiji remains uncertain. “We don’t truly understand what the needs are here. We must consider who will utilize the service, how they will be identified, when they can access it, and where it will be located, as well as the security measures for that area.”

Dr. Cikamatana acknowledged the serious risks associated with current practices among drug users, many of whom use drugs in public spaces. She stressed the urgent need for harm reduction strategies to be implemented.

The proposed needle exchange program would also necessitate a review and enhancement of existing laws concerning syringes. There is currently a new narcotics bill in development that could establish a narcotics court to help improve access to treatment and harm reduction services.

“One aspect of that bill includes the establishment of a narcotics court,” Dr. Cikamatana explained. “From that court, we will explore the pathways related to it.”

The ministry is dedicated to ongoing discussions to gauge the viability of the needle exchange program, keeping in mind the unique challenges faced in Fiji.
